Job summary

Evolve Therapy Services, LLC is hiring a Certified Occupational Therapy Assistant to provide in-house OT services at a Skilled Nursing Facility in Fostoria, OH.

The role requires an Associate's degree in Occupational Therapy, an active OTA licensure in the states of practice, and a commitment to ongoing continuing education.

You can work Full Time, Part Time, or PRN with competitive pay and benefits based on status.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention based on scope of practice.
• Perform treatment plans and follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings and recommendations to residents and care teams.
• Establish resident therapy schedules in cooperation with team members.
• Conduct therapy according to treatment plan and ensure proper documentation and billing.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily with point-of-care documentation.

Education

Associate's degree in Occupational Therapy

MUST have active OTA licensure in state(s) of practice

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time, or PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated
