Job summary

Evolve Therapy Services in Ohio is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility located in Fostoria. The role plans, organizes, develops, and directs Occupational Therapy Services to ensure the highest quality resident care while complying with federal, state, and local standards.

You will screen residents, implement treatment plans, document and bill services, and collaborate with families and the interdisciplinary team to optimize outcomes.

Qualifications

• Associate's degree in Occupational Therapy.
• MUST have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment per scope of practice.
• Perform treatment plans and follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team.
