Job summary

Evolve Therapy Services in Sutherland, NE is seeking a Certified Occupational Therapy Assistant to provide in-house SNF services on a PRN basis. You will plan, implement, and document OT interventions under supervision, ensuring compliance with CMS standards and resident care goals.

Responsibilities

• Conduct regular screening of residents to determine best interventions.
• Perform treatment plans following established goals.
• Interpret and communicate evaluation findings to residents, families, and team.
• Establish individual resident therapy schedules with the team.
• Conduct therapy per treatment plan and ensure proper documentation and billing.
• Comply with CMS documentation standards daily.
