Job summary

Evolve Therapy Services, LLC in Sutherland, Nebraska, is hiring a Certified Occupational Therapy Assistant (COTA) for an in-house Skilled Nursing Facility on a PRN basis. You will plan and direct therapy services under supervision, screen residents, develop and follow treatment plans, and collaborate with the care team to ensure quality resident care.

The ideal candidate will hold an Associate's degree in Occupational Therapy and an active OTA license in the practice states, with ongoing

• Associate's degree in Occupational Therapy required.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention per scope of practice.
• Perform treatment plans and follow goals per plan of care.
• Communicate evaluation findings and recommendations to residents and team.
• Establish therapy schedules with team members.
• Document and bill OT services in CMS-compliant manner.
• Perform additional duties as assigned and required to treat patients.
• Comply with CMS documentation standards daily.

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
