Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant to support an in-house Skilled Nursing Facility in Erie, PA. The role requires an active OTA license and an Associate's degree in Occupational Therapy, with ongoing continuing education to maintain licensure.

You will work under supervision to plan, implement, and document therapy services for residents, while collaborating with the multidisciplinary team.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention.
• Implement treatment plans and follow goals with supervision.
• Communicate evaluations and plans to residents and team.
• Schedule resident therapy with team input.
• Document and bill OT services per plan of care.
• Perform other duties to effectively treat patients.
• Adhere to CMS documentation standards daily.

Education
