Evolve Therapy Services is recruiting a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Massena, NY. Full Time, Part Time or PRN options available. The role requires an OTA license and an Associate's degree in Occupational Therapy.

Responsibilities include screening residents, developing and following care plans, collaborating with the interdisciplinary team, scheduling therapy, and ensuring accurate documentation and billing per CMS standards.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsabilités

• Screen residents to determine the best intervention per scope of practice.
• Perform treatment plans and progress toward goals per established plan of care.
• Interpret and communicate evaluation findings and plans to residents, families, and interdisciplinary team.
• Establish resident therapy schedules with other team members.
• Conduct therapy according to the established treatment plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards and daily point-of-care documentation.

Formation

Associate's degree in Occupational Therapy

Active OTA licensure in state(s) of practice

Ongoing continuing education and professional development
