Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility located in Kenosha, WI. This PRN position supports resident-centered OT services under the supervision of a licensed therapist.

You will screen residents, implement treatment plans, document outcomes, coordinate schedules, and collaborate with an inter-disciplinary team. Active OTA licensure in the state is required, with ongoing professional development encouraged.

Qualifications

• Active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.
• Ability to screen residents and plan therapy with supervision.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Perform treatment plan and follow goals established by supervising therapist.
• Interpret and communicate evaluation findings, plans, and recommendations to residents and families.
• Establish individual resident therapy schedules with team members.
• Conduct therapy and ensure proper documentation and billing of occupational therapy services.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily.
