Evolve Therapy Services is hiring compassionate therapists for our partner located in Freeport, IL. The Certified Occupational Therapy Assistant will provide in-house therapy services in a Skilled Nursing Facility and help deliver patient-centered care.

Position offers Full Time, Part Time or PRN options. Responsibilities include planning treatment, documenting care, and collaborating with a multidisciplinary team to meet resident goals.

Qualifications

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education to maintain licensure and certification required.
