Job summary

Evolve Therapy Services in Freeport, IL, seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. Roles include Full Time, Part Time or PRN positions to support resident care and therapy delivery.

You will help screen residents, implement treatment plans, communicate findings to families and the interdisciplinary team, and ensure proper documentation and billing under CMS guidelines.

Qualifications

• Associate's degree in Occupational Therapy.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention based on scope of practice.
• Implement treatment plans with short- and long-term goals.
• Interpret evaluation findings and communicate with residents, families/caregivers, and interdisciplinary team.
• Establish individual resident therapy schedules with other team members.
• Conduct therapy according to treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point of care documentation.

Education
