Evolve Therapy Services is hiring a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Freeport, IL. This PRN position supports resident care under supervision of a licensed therapist, focusing on functional outcomes and quality service.

You will conduct screenings, implement treatment plans, and document CMS-compliant notes while collaborating with an interdisciplinary team. Active IL OTA licensure is required.

Formación

• Associate's degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsabilidades

• Screen residents to determine best intervention or treatment.
• Implement treatment plans based on established goals with supervising therapist.
• Interpret evaluation findings and communicate with residents, families, and team.
• Establish resident therapy schedules with team members.
• Provide therapy and ensure proper documentation and billing per CMS standards.
• Perform other duties as required to treat patients.
• Maintain CMS documentation standards daily with point-of-care notes.
