Job summary

Evolve Therapy Services, LLC seeks a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Freeport, IL. Plan, implement, and monitor therapy services to residents under supervision of a licensed therapist.

Candidates must have an Associate’s degree in Occupational Therapy, active OTA licensure in Illinois, and commit to ongoing CE to maintain licensure. Full-time, part-time, or PRN positions available with CMS documentation compliance.

Qualifications

• Must hold active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention.
• Perform treatment plans under supervision of licensed therapist.
• Communicate evaluation findings and treatment plans to residents and families.
• Establish resident therapy schedules in collaboration with team.
• Document and bill for occupational therapy services per plan of care.
• Maintain CMS documentation standards daily.
