Job summary

Heritage Manor Care seeks a dedicated Occupational Therapy Assistant to join our post-acute care team. You will provide therapy services under supervision, contribute to the patient Plan of Care, and support our mission to deliver compassionate, high-quality care.

We value collaboration, ongoing education, and adherence to state guidelines. Required: Associate of Arts in OTA, state licensure, CPR certification, and 1 year OTA experience preferred; 1–2 years in residential or skilled nursing

Qualifications

• Associate degree in Occupational Therapy Assistant.
• Licensure as required by the State.
• CPR certification.
• 1 year OTA experience preferred; 1–2 years in residential/assisted living or SNF preferred.

Responsibilities

• Provides therapy services established by the OT in the patient Plan of Care.
• Provides therapy services under supervision of an OTR as determined by the Registered Therapist.
• Completes documentation including daily notes and assists in progress notes, discharge summaries, or monthly reports.
