Job summary

Centra Healthcare Solutions is seeking an experienced Certified Occupational Therapy Assistant (COTA) licensed or in the process of obtaining IL licensure to work in SNF/LTC settings in Rockford, IL.

This is a contract position with a 5x8 days schedule. The role requires ongoing accountability to integrate the organization's vision, mission, and goals into daily practice while delivering high-quality patient care.

Qualifications

• 1 year of recent experience as a Certified Occupational Therapy Assistant (COTA) in SNF/LTC.
• Current COTA license within the state of IL or in the process of obtaining licensure.

Responsibilities

• Provide occupational therapy services to patients in SNF/LTC settings.
• Collaborate with care teams to implement patient-centered treatment plans.
• Document evaluations and progress in accordance with regulatory requirements.
