Concept Rehab is hiring a Certified Occupational Therapy Assistant (COTA) to join our team at a skilled nursing facility in Avon, OH. Flexible Part-Time and PRN schedules provide stability, flexibility, and supplemental income for clinicians.

As a COTA, you will assist in ADL-focused therapy, collaborate with a multidisciplinary team, and support discharge planning. New graduates are welcome to apply, and Ohio licensure is required or eligible.

What You’ll Do

As a Certified Occupational Therapy Assistant in a Skilled Nursing Facility, you will help patients improve independence with daily living skills following illness, injury, or hospitalization.

Key Responsibilities:

• Deliver occupational therapy treatments under the supervision of an OTR
• Implement individualized care plans focused on ADLs and functional independence
• Collaborate with therapists, nursing staff, and families
• Maintain accurate and timely electronic documentation
• Educate patients, caregivers, and staff on therapy programs
