Job summary

Shadelands Post Acute in Walnut Creek, CA is seeking a Certified Occupational Therapy Assistant (COTA) for PRN hours to join our rehabilitation team. You will implement individualized treatment plans under the supervision of a licensed OT, help patients improve ADLs and functional mobility, and contribute to progress documentation and care conferences.

  • Current and active COTA certification and state license.
  • Graduate of an accredited COTA program.
  • Strong communication and interpersonal skills.

Responsibilities

  • Provide occupational therapy treatment under the direction of a licensed OT.
  • Implement individualized treatment plans based on patient needs and goals.
  • Assist patients in improving independence with ADLs and functional mobility.
  • Provide therapeutic activities to improve strength, coordination, balance, cognition, and functional skills.
  • Monitor and communicate patient progress to the OT and interdisciplinary team.
  • Complete accurate documentation in line with policies and payer requirements.
  • Educate patients and families on home programs and safety strategies.
  • Participate in care conferences and discharge planning as needed.
  • Maintain compliance with state regulations and facility policies.
  • Build positive relationships with patients, families, and coworkers.
