Sylvan Shores Health & Wellness seeks a Certified Occupational Therapist Assistant (COTA) for our in-house rehabilitation program in Texas. The role focuses on implementing OT care plans, coordinating with the care team, and educating patients and families to support meaningful recovery outcomes.

The candidate should be Texas-licensed, with SNF or Veterans Home experience preferred, and possess strong communication skills and a collaborative mindset to deliver high-quality patient care.

Qualifications

• Certified Occupational Therapist Assistant (COTA) in the State of Texas.
• Experience in Skilled Nursing or Veterans Home settings preferred.
• Knowledge of SNF or Veterans Healthcare regulations and general healthcare compliance.
• Strong communication skills (written, verbal and non-verbal).
• Autonomous and self-motivated with a team-based approach to problem solving.

Responsibilities

• Facilitate rehabilitation of patients with emotional, physical or mental disabilities.
• Implement occupational therapy care plans.
• Coordinate care with the entire care team.
• Educate patients and family members.
• Prepare timely, complete and accurate clinical notes.
• Communicate effectively with patients, caregivers and other care team members.
