The Gardens at West Shore, a premier skilled nursing facility, is seeking a dedicated COTA to join our rehabilitation team. You will provide exceptional therapy services, promote independence, and enhance residents' quality of life under the supervision of a licensed Occupational Therapist (OT).

Key Responsibilities

1. Deliver occupational therapy services in accordance with standards, procedures, and guidelines, as directed by the OT.
2. Conduct resident screenings regularly to assess intervention needs, complying with state laws and policies.
3. Assist in developing personalized treatment plans with the OT.
4. Maintain accurate documentation and communicate progress to the OT and team.
