Location: Yoakum Nursing and Rehabilitation Center - Yoakum, TX

Position Type: PRN

Travel Percentage: Up to 50%

Description

The Certified Occupational Therapy Assistant (COTA) will implement the plan of care as established by the therapist and physician, maintaining a high standard of quality care. The COTA will communicate with facility staff and support the facility through training and in-services as needed.

Qualifications

• Screen residents quarterly to identify declines in functional abilities and determine if therapeutic intervention is needed.
• Evaluate residents per MD orders and develop treatment plans and goals tailored to individual needs.
• Re-assess patients regularly to decide on continuation or discontinuation of services based on progress.
• Carry out treatment programs as indicated.
• Provide comprehensive discharge planning to maximize patient independence in the least restrictive environment.
• Educate staff, families, and caregivers as appropriate.
• Identify and implement program development needs according to company policies.
• Determine equipment needs and order accordingly.
• Complete documentation following specified guidelines.
• Coordinate in-services for staff to ensure safe and effective care and maintenance of programs.
• Attend all required meetings.
• Supervise therapy assistants/techs per state regulations.

Licensing Requirements

The COTA must hold an active Texas state licensure for Therapy Assistant.
