Job summary

Garden Spring Rehab and Care Center in Willow Grove, PA is hiring a full-time Occupational Therapy Assistant to join our compassionate health team.

You will assess referred patients, establish and monitor rehabilitation plans, document progress, and educate patients and families under physician direction to promote optimal recovery and well-being.

A PA license/registration and graduation from an accredited OTA program are required; experience in long-term care or geriatric settings is preferred.

Qualifications

• Graduate of an accredited OTA program with state license.
• Valid PA license/registration required.
• Experience in long-term care or geriatric care preferred.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Establish and administer treatment programs with goals per patient capacity.
• Set realistic goals and document patient condition and progress.
• Educate patients and families in therapy methods; maintain progress notes and discharge summaries.
• Collaborate with disciplines to provide patient-centered care; comply with confidentiality and rights.

Duties and Responsibilities

Assessment of referred patients that require rehabilitation services

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements

Must have graduated from an accredited college or university.

• Must have a valid professional license and registration for state of PA
• Experience with long-term care and/or geriatric care is preferred but not limited to.
