Job summary

Personal Healthcare Management LLC is seeking a licensed Occupational Therapy Assistant in New York. The role requires an AS degree in Occupational Therapy and a current NYS license, with geriatric experience preferred.

Qualifications

• Must possess a current unencumbered NYS license as an Occupational Therapy Assistant.
• Graduate of an accredited school with an Associate’s degree in Occupational Therapy.
• Geriatric experience preferred.
• A valid driver’s license and reliable transportation.
• Ability to professionally communicate with internal and external resources.

Responsibilities

• Provide OT treatment plans and modalities per physician orders.
• Communicate tactfully with residents, families, staff, and the public.
• Address residents with visual, speech, cognitive, and physical impairments.
• Train, supervise, and discipline subordinate staff as needed.
• Assist in planning, organizing, and implementing department goals and policies.
• Document resident progress toward OT goals and complete daily notes.
• Educate residents, staff, and families as directed.
• Ensure compliance with Federal and State rehab regulations.
• Utilize clinical software per policies.
• Follow all facility policies and maintain confidentiality.
• Report potential resident abuse, neglect, or misappropriation immediately.
• Maintain infection control practices.

Essential Job Functions:

• Provides Occupational Therapy plan of treatments and modalities per physician orders
• Must possess the ability to deal tactfully and effectively communicate with personnel, residents, family members, visitors, and the general public
• Must effectively communicate with residents with visual, speech, auditory, cognitive, physical and emotional impairments
• Must possess the ability to train/supervise and discipline subordinate staff
• Must possess the ability to assist in planning, organizing and implementing the goals, objectives and policies and procedures of the department
• Documents resident progress toward occupational therapy goals
• Completes Daily Activity Schedule and progress notes per facility policy.
• Provides resident, staff and family education as directed
• Complies with Federal and State regulations related to rehabilitation therapy
• Utilizes the clinical software applications per policies and procedures
• Complies with all facility policies and procedures
• Maintains confidentiality of facility and resident information
• Reports all instances of potential resident abuse, neglect, and misappropriation of resident property immediately
• Follows sanitation and infection control policies and procedures
