Job summary

Personal Healthcare Management LLC in New York is seeking a licensed Occupational Therapy Assistant to provide therapy services per physician orders. You will work with residents, families, and staff, delivering targeted treatment plans and progress documentation throughout the shift.

The role emphasizes supervising staff, maintaining regulatory compliance, and using clinical software while upholding confidentiality and high-quality care standards.

Qualifications

• Must hold a current unencumbered NYS license as an Occupational Therapy Assistant.
• Graduate of an accredited program with an associate's degree in occupational therapy.
• Geriatric experience preferred.
• Valid driver's license and dependable transportation.
• Ability to communicate professionally with internal and external resources.

Responsibilities

• Provides Occupational Therapy plan of treatments and modalities per physician orders.
• Must communicate tactfully with residents, families, staff, and the general public.
• Communicate effectively with residents with visual, speech, auditory, cognitive, physical and emotional impairments.
• Train/supervise and discipline subordinate staff.
• Assist in planning, organizing and implementing department goals, objectives and policies.
• Documents resident progress toward Occupational Therapy goals.
• Completes Daily Activity Schedule and progress notes per facility policy.
• Provides resident, staff and family education as directed.
• Complies with Federal and State regulations related to rehabilitation therapy.
• Utilizes clinical software applications per policies and procedures.
• Follows all facility policies and maintains confidentiality of information.
• Reports all instances of potential resident abuse, neglect, and misappropriation of resident property immediately.
• Follows sanitation and infection control policies and procedures.
