Job summary

Trinidad Rehabilitation & Healthcare Center is seeking a compassionate Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team in Trinidad, CO. This full-time role offers free housing and relocation assistance along with competitive pay.

You will provide OT services under a licensed OT, implement individualized plans, and document resident progress in a skilled nursing and long-term care setting. Join a supportive team focused on resident independence and quality of life.

Qualifications

• Current Colorado COTA license or eligibility for Colorado licensure.
• Graduate of an accredited Occupational Therapy Assistant program.
• Experience in Skilled Nursing Facility (SNF) or Long-Term Care (LTC) preferred.
• Excellent communication and documentation skills.
• Passion for providing exceptional resident-centered care.

Responsibilities

• Provide occupational therapy services under the direction of a licensed Occupational Therapist in a skilled nursing and long-term care setting.
• Assist with implementing individualized treatment plans to help residents achieve their highest level of independence.
• Accurately document treatments and resident progress.
• Collaborate with the interdisciplinary care team to maximize resident outcomes.
• Follow all state and federal regulations.
• Adhere to facility policies and procedures.
• Perform other duties as assigned.
