Job summary

Vineyard Post Acute in Petaluma, CA is seeking a dedicated Certified Occupational Therapy Assistant (COTA) to join our rehab team on a part-time basis. You will provide direct patient treatments under the supervision of an Occupational Therapist and support high-quality rehab services.

Qualifications

• Strong documentation and computer skills are required.
• Must maintain licensure in good standing and meet CE requirements.
• Preparation and collaboration with OT and rehab team.

Responsibilities

• Provide treatments under the supervision of an OT.
• Document daily treatment notes and progress notes.
• Follow physician treatment plan and facility protocols.
• Assist with training Restorative Aides as needed.
• Communicate patient progress to supervising OT and care team.
• Participate in in-service training and interdisciplinary meetings.
• Educate patients and families on maintenance programs.
• Support discharge planning.
• Help maintain a clean and organized therapy space.
• Report equipment issues to ensure safety.

Role Summary

The COTA provides direct patient treatments under the supervision of a licensed Occupational Therapist and supports the delivery of high-quality rehab services.

Essential Duties

• Provide treatments as directed by the Occupational Therapist
• Document daily treatment notes, progress notes, and recertification documentation per state and OT Board requirements
• Follow the physician treatment plan and facility protocols
• Assist with training Restorative Aides as needed
• Communicate patient progress and concerns to the supervising OT and care team
• Participate in in-service training and interdisciplinary meetings
• Educate patients, families, and nursing staff on maintenance programs and caregiver strategies
• Support discharge planning
• Help maintain a clean and organized therapy space
• Report equipment issues to ensure safe, proper functioning
• Build positive, professional relationships with residents, families, and staff

Qualifications

• Licensed Occupational Therapy Assistant in the State of California
• Strong documentation and computer skills (Casamba, PointClickCare preferred)
• Meets all continuing education requirements and maintains license in good standing
