Job summary

Horizons Care Center seeks a compassionate CNA to provide hands-on care and support to residents in a skilled nursing and rehabilitation facility. You will assist with daily living activities, mobility, personal hygiene, and meals while closely observing and reporting changes in residents’ conditions.

You will collaborate with an interdisciplinary team to carry out personalized care plans, uphold safety and infection control standards, and promote dignity and quality of life for each resident.
