Job summary

Jobtailor is seeking a compassionate Certified Nursing Assistant to join our long-term care team. You will provide resident-centered care under RN supervision, assist with daily living activities, and help implement care plans.

Travel to multiple locations within 75 miles, weekend shifts monthly, and ongoing certification maintenance are required. Candidates must have CNA certification and may pursue QMAP where required. The role emphasizes infection control, documentation, and teamwork.

Qualifications

  • Travel required to support multiple locations within 75-mile radius of permanent address.
  • 3 months experience in Long Term Care required (6 months preferred).
  • Minimum of 30 hours per month.
  • One weekend shift per month.
  • Minimum of 18 years of age.
  • Required CNA certification with the State Board of Nursing, or state certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement.
  • CNA training and certification required and provided by facility.
  • Colorado: Depending on facility, QMAP certification with the Colorado Department of Public Health & Environment required.
  • Satisfactorily completes yearly department specific competency testing.
  • Maintains a record of continuing education credits; a minimum of 12 hours continuing education are required annually.
  • Maintains all department specific required certifications.
  • When applicable, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
  • Depending on facility and required responsibilities, may classify as a Sanford Category II Driver requiring a motor vehicle report and proof of valid driver’s license.
  • Must be medically qualified to perform essential driving functions when applicable.

Responsibilities

  • Serve as caregiver to residents during scheduled work periods in long-term care.
  • Provide resident-centered nursing care and daily living assistance under RN supervision.
  • Deliver age-appropriate care according to care plans.
  • Follow infection prevention, control policies, and Personal Protective Equipment procedures.
  • Assist with bathing, toileting, grooming, dressing/undressing, vital signs, psychosocial support, and personal care.
  • Assist residents with transferring, repositioning, walking, range-of-motion, and passive exercises.
  • Complete food preparation and dining tasks; assist with meals, snacks, water, and supplements.
  • Aid providers and nursing staff with procedures as needed.
  • Document resident interactions and outcomes related to care, behavior, activity, and dietary needs.
  • Communicate changing resident conditions and care concerns to the charge nurse.
  • Collect non-invasive body fluid specimens and vital signs.
  • Provide housekeeping and laundry services.
  • Support wellness and recreational programs and encourage resident participation.
  • Participate in departmental or organizational committees and task forces as needed.
