Job summary

Elara Caring is seeking a Certified Nurse Aide to provide compassionate home-based care to patients. You will perform personal care tasks, assist with daily activities, monitor vital signs, and support safe mobility under the supervision of a Registered Nurse.

Qualifications

Must be a Certified Nursing Aide with active CNA credential.
Ability to frequently lift, push, pull and support up to 50 pounds.
Must possess a valid drivers license, or have reliable transportation to travel between patient homes.

Responsibilities

Provide hands-on personal care under RN supervision.
Assist patients with bathing, dressing, grooming, toileting, and daily activities.
Take and record vital signs (temperature, pulse, respirations, blood pressure).
Help patients safely walk, transfer, and use mobility equipment.
Follow each patients individualized care plan.
Monitor and report changes to the care team.
Maintain a clean, safe home environment.
Remind patients to take medications as directed.
Respond to emergencies and follow infection-control guidelines.
Maintain accurate documentation and protect patient privacy.
