Job summary

Elara Caring is seeking a Certified Nurse Aide to join our home care team, providing essential personal care to patients in their homes under RN supervision.

You will assist with daily activities, monitor vital signs, and follow each patient’s care plan while ensuring safety and privacy. This role requires mobility, reliability, and compassionate patient support across assigned locations.

Qualifications

• Current State certificate as a Certified Nursing Aide.
• Ability to frequently lift up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license or have reliable transportation to travel between patient homes and work locations.

Responsibilities

• Provide hands-on personal care under the direction of a Registered Nurse.
• Assist patients with bathing, dressing, grooming, toileting, and other daily activities.
• Take and record vital signs, including temperature, pulse, respirations, and blood pressure.
• Help patients safely walk, transfer, and use mobility equipment.
• Follow each patient's individualized care plan.
• Monitor and report changes in a patient's condition to the care team.
• Maintain a clean, safe, and comfortable home environment.
• Remind patients to take medications as directed.
• Respond to emergency situations and follow safety and infection control guidelines.
• Maintain accurate documentation and protect patient privacy.

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
