Job summary

Brighton Hospice is seeking a dedicated Certified Nursing Assistant/Home Health Aide (CNA/HHA) to join our compassionate Care team. Under the Nursing Leadership Team, you will provide personalized hospice care to patients in their homes or facility, supporting assessments and care plans with dignity and respect.

You will collaborate with nurses, social workers, chaplains and volunteers; travel as needed with reliable transportation; maintain infection-control practices and company policies.

Qualifications

• Compassionate and committed to improving patients’ quality of life.
• Ability to work collaboratively with Nurses, Social Workers, Chaplains, Volunteers, and Aide Leadership.
• Comfortable working one-on-one with patients while being supported by a dedicated care team.
• Willingness to travel with reliable transportation, a valid driver’s license, and current auto insurance.
• Current CNA certification in the state(s) of practice.
• Current HHA certification in the state(s) of practice.
• Eagerness to pursue additional certifications, education, and career growth opportunities.

Responsibilities

• The CNA/HHA collaborates with the hospice care team to meet patient needs, promote comfort and dignity, and report changes in condition.
• The CNA/HHA follows all Company policies, procedures, safety guidelines, and infection-control practices.
