About the job:

Pay: $17-$21 based on experience

The Home Care CNA provides assistance with activities of daily living and supportive care to ensure the health and safety of participants in their home environment. Responsibilities include monitoring health status, providing a safe environment, and assisting with day program activities at the SCC Day Center and participant homes.

Required Qualifications:

• Active and up-to-date CPR certification
• Minimum of one year experience working with the elderly or frail population
• High School diploma or equivalent
• Medical clearance for communicable diseases and up-to-date immunizations
• Valid driver’s license, proof of insurance, and transportation for home visits

Preferred Qualifications:

• Current STNA license
• Bilingual skills

Additional Responsibilities:

• Consult daily with the Home Care Coordinator regarding participant status and care plans
• Assist participants with personal hygiene, ambulation, prescribed exercises, meals, bathroom needs, medication assistance, and other health-related tasks
• Perform household and homemaker services
• Assist in the Day Center with activities of daily living and monitor participant meals
• Maintain cleanliness and assist with laundry and transportation
