Job summary

Bradford Hills Nursing & Rehabilitation Center seeks a compassionate Certified Nursing Assistant for the evening shift to provide direct patient care in a supportive setting. Duties include vital sign checks, ADL assistance, and coordinated care with the interdisciplinary team.

Qualifications

About the Role:

• Providing direct patient care, including vital sign checks, assistance with activities of daily living, and medication administration
• Working collaboratively with the interdisciplinary care team to develop and implement individualized care plans
• Maintaining accurate and detailed records of patient care and treatment
• Assisting with resident transfers, ambulation, and mobility exercises
• Participating in rounds and reporting any changes or concerns to the assigned nurse

Requirements:

• Current certification as a Certified Nursing Assistant (CNA) in the state of Pennsylvania
• High school diploma or equivalent required
• Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
