Qualifications

• Drivers License - Licensing Board Upon Hire
• Certified Nurse Aide (CNA) - Licensing Board If working in the state of Minnesota Upon Hire
• High School or GED Equivalent
• Certified Nurse Aide (CNA) - Licensing Board An active license in the state of practice
• Less than 1 year in home health care or related health care field

Responsibilities

• Provides assistance to patients with activities of daily living.
• Provides personal care as directed by the nurse per the plan of care.
• Provides housekeeping, laundry, meals, and shopping.
• Assists patient with social activities per care plan.
• Maintains timely and accurate documentation.
• Reports changes in patient status to supervisor.
• Uses equipment and supplies efficiently and safely.
• Adheres to safety and infection control policies.
