Job description

Haverhill Rehabilitation and Healthcare Center IS NOW OFFERING $1,000 SIGN ON BONUS!!!!


Patient Care Requirements:

  1. Identifies special resident problems and changes in condition of resident and report them immediately to Charge Nurse.
  2. Bathes, neatly dresses (in their own clothing) and undresses residents.
  3. Grooms resident including shaving, hair and fingernail care.
  4. Accurately records percentage of resident food and fluid intake.
  5. Feeds resident according to care plan. Monitors meal tray.
  6. Answers call light.
  7. Delivers and removes trays.
  8. Pulls up side rails and rolls beds down.
  9. Changes incontinent residents.
  10. Takes residents to bathroom every two hours and as requested.
  11. Makes sure all residents are offered nourishment as assigned.
  12. Completes tub baths, showers or bed baths.
  13. Turns and repositions all bed and chair residents every two hours including pulling up in bed.
  14. Empties and records all urinary drainage units.
  15. Applies restorative devices such as splints, handrails, braces, eyeglasses and hearing aids.
  16. Utilizes techniques to manage physically and verbally abusive residents.
  17. Monitors and redirects confused wandering residents.
  18. Assists nurse with treatment procedures.
  19. Changes bed linens on occupied and unoccupied beds.
  20. Performs oral hygiene and denture care for residents.
  21. Takes records and reports temperature, pulse, respiration rate and blood pressure.
  22. Takes records and reports weights of residents.
  23. Accurately uses different types of scales i.e. chair, hoyer, stand-up.
  24. Performs routine foot care.
  25. Applies creams and lotions to prevent skin breakdown, including back rubs.
  26. Inspects resident skin condition with daily care and reports to the charge nurse.
  27. Transfers the resident from bed to chair and chair to bed.
  28. Assists resident with use of bedpan or urinal.
  29. Provides perineal care to male and female residents.

Safety & Health Requirements:

  1. Identifies safety hazards and emergency situations and reports appropriately.
  2. Follows facility fire, safety and evacuation plans.
  3. Functions effectively in emergency situations.
  4. Follows facility procedures related to control of equipment and supplies within the unit. Assures that work/assignment areas are clean and equipment and supplies are stored before leaving care area on breaks and at end of shift.
  5. Places bedpans, linen, washbasins and soiled linen in proper area.
  6. Observes infection control procedures including hand washing and isolation procedures.
  7. Assumes responsibility for the safety of residents under his/her supervision, including resident alarms being on and intact when ordered for resident.
  8. Performs resident alarm checks when assigned per facility protocol.
  9. Observes all facility safety policies and procedures.
  10. Utilizes proper body mechanics for self and residents including use of gait belts and other equipment.
  11. Transports residents in required equipment safely to other departments or activities.
  12. Maintains residents' units - bedside table, bureau, closets, and personal care items in a clean and orderly fashion.
  13. Knows and uses universal precautions.

Individual Growth Requirements:

  1. Participates in facility education programs as assigned.
  2. Attends all classes as assigned and completes assignments.
  3. Comes to work in a clean neat uniform and consistently presents a professional appearance in accordance with department dress code.
  4. Comes to work as scheduled and consistently demonstrates dependability and punctuality.
  5. Accepts assigned duties in a cooperative manner.
  6. Assists others when own work is completed.
  7. Effectively organizes varying workloads.

Documentation Requirements:

  1. Documents incidents and/or unusual problems according to established facility procedures. i.e. personal injuries within 24 hours; residents incidents immediately.
  2. Documents in the nursing assistant notes the care and treatment provided to the resident and the resident's response or lack of response to care provided.
  3. Documents positioning and restraint usage on designated flow sheets.

General Requirements:

  1. Follows established performance standards and performs duties according to nursing service policies and procedures.
  2. Assumes accountability for compliance with federal, state and local regulations within the unit assigned and within his/her span of control.
  3. Listens tolerantly and patiently to resident and family complaints and report problems to Charge Nurse.
  4. Follows Resident's Rights policies at all times including confidentiality, privacy and dignity.
  5. Consistently works cooperatively with residents, charge nurses, staff nurses, restorative assistants, other nursing assistants, physicians, families, consultant personnel, ancillary service providers and administration.
  6. Performs other related duties as directed by licensed nursing personnel.
  7. Accepts assignments to specific areas of nursing home at the discretion of management.

NON-ESSENTIAL FUNCTIONS:

  1. Assists new employees in following established facility policies and procedures.
  2. Participates in resident care conferences and other facility meetings as assigned.
  3. Reviews care plans and performs nursing care as outlined.
  4. Follows facility procedures for admission, discharge, and transfer of residents.
  5. Performs post mortem care according to facility policy and procedure.
  6. Accompanies patients to appointments out of facility.
