Job summary

Signature HealthCARE LLC in Prestonsburg, KY, is hiring a Certified Nursing Assistant (PRN) for day shifts (6:00 AM–6:00 PM). You will deliver direct resident care under licensed staff, support ADLs, monitor vital signs, and assist in training other team members.

This position emphasizes compassionate, patient-centered care within a values-driven culture. Excellent communication and teamwork are essential, with a focus on maintaining resident dignity and comfort.

Qualifications

• Current, active Nursing Assistant certification from the state.
• Excellent verbal and written communication skills.
• Ability to work collaboratively within a nursing team and under licensed nursing staff.
• Customer service oriented with ability to work under pressure.
• Approachable, flexible, and adaptable to change.
• Function independently with integrity and stakeholder interaction.

Responsibilities

• Provide direct resident care duties under supervision of licensed nursing personnel.
• Assist with activities of daily living (ADLs) per care plan.
• Change bed linens and maintain supplies in resident rooms.
• Ambulate and transfer residents using assistive devices.
• Record vital signs, weights, intake/output, and other data as assigned.
• Routinely turn and position residents and support rights and comfort.
• Maintain housekeeping standards in duty areas and respond to call lights.
• Assist in orientation and training of other staff as requested.
• Perform other duties and projects as assigned.

How you Will make a difference

Perform direct resident care duties under the supervision of licensed nursing personnel. Assist with promoting a compassionate physical and psychosocial environment for the residents in accordance with facility policies and procedures.

• Provide activities of daily living (ADLs) within the scope of practice as defined by state regulation and the resident’s plan of care.
• Change bed linen and maintain appropriate grooming supplies in resident room on a regular basis.
• Ambulate and transfer residents, utilizing appropriate assistive devices and body mechanics.
• Record resident vital signs, weights and measurements, intake and output, food consumption, and other information, as assigned.
• Routinely turn and position residents, as necessary.
• Recognize and support resident rights and choices within the care environment.
• Maintain established housekeeping standards within assigned duty areas.
• Promptly answer call lights and assist with resident needs.
• Assist in the orientation and training of other staff, as requested.
• Other special projects and duties, as assigned.

What you Need to make a Difference

• Current, active Nursing Assistant certification from state in which the facility operates.
• Effective verbal and written communication skills.
• Ability to work collaboratively within a nursing team and under the direction of licensed nursing staff.
• Customer service oriented with the ability to work well under pressure.
• Approachable, flexible, and adaptable to change.
• Function independently, and have flexibility, personal integrity, and the ability to work effectively with stakeholders and vendors.
