Job summary

AdventHealth Home Health and Hospice is seeking a compassionate CNA to provide essential in-home personal care, mobility assistance, and daily living support in Altamonte Springs and surrounding areas.

You will document visits in the EMR, coordinate with clinicians, and participate in team meetings. A valid CNA, active BLS, and a driver’s license are required; CHPN is preferred, with travel to patient homes as part of the role.

Qualifications

• High School grad or equivalent.
• Certified Nurse Assistant (CNA) required.
• Driver's license required.
• Basic Life Support (BLS) certification required.
• CHPN preferred.
• Knowledge of electronic medical record (EMR).

Responsibilities

• Provide personal care to patients, including bathing, grooming, ADLs.
• Assist with mobility and range-of-motion activities.
• Keep patient areas clean and linens changed; assist with laundry.
• Assist with meals and hydration and monitor intake.
• Document visits in patient records and communicate with care team.

Monday - Friday with rotating weekends. Shift: Dayshift 8:30am-5pm. Location: Covering Longwood, Altamonte Springs, Winter Springs, Oviedo and Maitland

Job Description:

• Provides personal care to patients, including bathing, shampoo, nail and foot care, oral hygiene, and assisting with Activities of Daily Living (ADL).
• Aids patient mobility and performs range of motion activities as outlined in the plan of care.
• Cleans and maintains the patient area, changes linens, and does patient laundry as needed.
• Prepares and serves meals or snacks, and assists patients with eating or drinking as needed.
• Leaves necessary supplies for patients, including creams, lotions, and briefs.
• Communicates with patients to assess their living and body conditions, ensuring a safe environment.
• Reviews plans of care for assigned patients and communicates with appropriate personnel regarding activities performed and patient conditions.
• Discusses patient care and needs with family members and responds appropriately to difficult or emotional situations.
• Documents each patient visit and activities performed in the patient database system, noting recommendations for other disciplines if needed.
• Participates in staff, department, and team meetings, as well as in-service and other related activities.
• Travels to homes, skilled nursing facilities, or assisted living facilities to provide personal care as required by the plan of care.
• Other duties as assigned.

Knowledge, Skills, and Abilities:

• Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements.
