Job summary

AdventHealth Home Health and Hospice Inc in Florida seeks an experienced Registered Nurse for a full-time night shift. The position involves home visits across Seminole, Orange and Osceola Counties, mileage reimbursement, and a company-provided phone and laptop/iPad.

You will coordinate with triage and case management, ensure timely documentation, and participate in interdisciplinary team meetings. The ideal candidate has 2+ years RN experience, an active RN license, and strong knowledge of EMR

Qualifications

• Associate's of Nursing required; Bachelor's preferred.
• RN license required; CPR/BLS certification required.
• 2+ years RN experience; home health/acute care preferred.
• Valid driver's license and auto insurance required.

Responsibilities

• Communicates patient and staff information to care teams daily.
• Travels to patient residences to assess conditions as needed.
• Provides nursing care, adminsters medications, and documents care.
• Responds to emergencies and supports the inpatient unit when needed.

Job Description

Schedule: Full Time Shift: Dat Shift, 9a-9p Every Saturday, Sunday and one additional day for RN After Hours Shift: Night Shift, 6:45p-7am with every other weekend for Specialized Staffing Location: Covering Seminole, Orange and Osceola Counties Mileage reimbursement provided: $0.76 per mile Company provided phone and laptop/iPad

Communicates pertinent patient and employee information to interdisciplinary teams and Patient Care Managers daily. Consults with the Triage Nurse or on-call Nursing Manager regarding unusual situations and potential risk management issues before initiating Crisis Care or approving General Inpatient (GIP) status. Completes all documentation regarding patient contact activities timely and ensure accuracy and completion. Participates in staff meetings, department meetings, team meetings, in-services, and other related activities. Responds in a timely manner to emails and phone calls during assigned hours of duty. Assists in the Inpatient Unit and provide nursing care to patients as needed. Responds to patient emergencies outside normal business hours and during early morning hours. Travels to patient residences as directed by the Triage Nurse to assess patient conditions based on emergency or urgent needs. Assesses patient conditions and need for immediate care due to falls or other traumatic events, communicating with Triage and Case Management staff. Performs nursing procedures such as inserting catheters, feeding tubes, and administering intravenous medications if credentialed. Educates the patient's family and other caregivers on hospice care and services provided to the patient. Coordinates patient needs for medications and medical equipment with the Triage Nurse, delivering supplies and prescriptions as needed. Other duties as assigned.

Knowledge, Skills, and Abilities

• Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements. [Required]
• Knowledge of electronic medical record. [Required]
• Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee. [Required]
• Knowledge of basic to intermediate application of Microsoft Office Suite including, Word, Excel, PowerPoint, Outlook as well as internet software. [Required]
• Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures and other instructions furnished in written, oral, diagram, or schedule form. [Required]
• Ability to follow standard precaution guidelines for infection control, including when providing patient care. [Required]
• Ability to complete AdventHealth Hospice Care CFL Orientation and Training, as necessary. [Required]
