Transitions Hospice in Peoria, IL seeks a Certified Nursing Assistant (CNA) to join our home-based care team. The role provides personal care, compassionate support, and eyes on vital signs within patients' homes, with a schedule Monday through Friday (8-hour shifts) and on-call rotation.

• Active CNA license or certification in the state of employment
• Current CPR certification
• 1+ year hospice, home health or related experience preferred
• Valid driver''s license and willingness to travel locally for patient visits

Responsabilidades

• Providing personal care with dignity, comfort, and respect at the center of everything
• Attending to patient needs and requests promptly and compassionately
• Monitoring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed
• Observing patients and reporting changes to the care team accurately and in a timely manner
• Communicating with patients and families in a positive, ethical, and confidential manner
• Maintaining thorough documentation in line with organizational standards
• Upholding the hospice philosophy with professionalism and genuine compassion

Skills, Knowledge and Expertise

What You Bring

• Active CNA License or Certification in the state of employment
• Possess and maintains current CPR certification
• One (1) + year Hospice, home health or related organization experience preferred
