Transitions Hospice seeks a compassionate Certified Nursing Assistant (CNA) to join our patient-focused team in McHenry, IL. The role provides in-home care, supporting patients and families with dignity, comfort, and respect, Monday through Friday 8am–4pm, with on-call rotations.

You will perform personal care, monitor vital signs, document changes, and coordinate with the interdisciplinary team. A current CNA license and CPR certification, plus a willingness to travel locally, are required;

Formación

• Active CNA License or Certification in the state of employment.
• Possess and maintains current CPR certification.
• One (1) + year Hospice, home health or related organization experience preferred.
• Comfort and sensitivity working with terminally ill patients and their families.
• A calm, dependable presence under emotionally demanding circumstances.
• Strong attention to detail in observation and documentation.
• A valid driver's license and willingness to travel locally for patient visits.

Coverage Area: McHenry, Crystal Lake, Woodstock, Fox Lake and surrounding areas

Salary: $22.00 - $25.00 / hour

Schedule: M-F 8am - 4pm with on call and rotating holidays
