Transitions Hospice is seeking a Certified Nursing Assistant (CNA) in Rockford, IL for a full-time role. You will provide compassionate in-home care, monitor vital signs, and support patients and families as part of an interdisciplinary team.

Responsibilities include personal care with dignity, timely communication with the care team, accurate documentation, and respecting patient preferences. A valid CNA license and CPR are required, with patient-home visits across the Rockford area.

Formación

• Active CNA License in the state of employment.
• Current CPR certification.
• 1+ year Hospice or home health experience preferred.
• Comfort with terminally ill patients and families.
• Calm, dependable presence under emotionally demanding circumstances.
• Strong attention to detail in observation and documentation.
• Valid driver’s license and willingness to travel locally for patient visits.

Description

Coverage Area: Rockford, Machesney Park, Belvidere and surrounding areas

Salary: $22.00 - $25.00 an hour

Schedule: Mon-Fri (8 hour shifts) w/rotating on-call weekends & rotating holidays

What You Bring

• Active CNA License or Certification in the state of employment
• Possess and maintains current CPR certification
• One (1) + year Hospice, home health or related organization experience preferred
