Transition Home Healthcare seeks a dedicated CNA for PRN shifts delivering in-home and hospice care. You will collaborate with the nursing team, maintain patient records, assist with meals and activities of daily living, and provide compassionate support to patients and families.

You will also review patient conditions, prepare clinical notes, and communicate with providers. Requires Texas CNA license, 1 year home health experience, ability to lift 50 pounds, and availability for

Competenze

• CNA license in Texas is required.
• One (1) year home health experience.
• Able to lift up to fifty pounds.
• Able to work occasional weekends, holidays, evenings.
• Strong communication and people skills.
• Experience with HCHB EMR is preferred but not required.
