Job summary

Transition Home Healthcare is seeking a PRN CNA in Texas to support home health and hospice services across multiple coverage areas. You will collaborate with the nursing team to deliver quality care, document patient progress, and assist with daily living activities.

Strong communication is essential, with weekend/evening availability expected. Applicants should hold a Texas CNA license and have at least 1 year of home health experience; lifting up to 50 pounds is required.

Qualifications

• CNA license in Texas is required.

Responsibilities

• Coordinate with the assigned nursing care team to provide exceptional patient care.
• Maintain patient records including changes in orders, diet, and activities of daily living.
• Assist with meals, feeding, and ADLs as needed.
• Deliver weekly reports to the case manager and note changes in patient conditions.
• Educate patients and family members about care plans.
• Prepare timely, complete clinical notes.
• Communicate effectively with healthcare providers and care team members.

CNA (Home Health & Hospice)

Shift Available: PRN

Service Areas for Coverage: North, Northeast, Northwest, Southwest

Responsibilities:

• Provide exceptional care in coordination with the assigned nursing care team.
• Maintain patient records that include any changes in doctor's orders, diet, or personal living activities.
• Help with patient meals, feeding, and basic activities of daily living.
• Deliver weekly reports to the case manager and review any important changes in patient conditions.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with healthcare providers and other care team members.

Qualifications:

• CNA license in the state of Texas (required).
• One (1) year home health experience.
• Able to lift up to fifty (50) pounds.
• Able to work occasional weekends, holidays, evenings.
• Strong communication and people skills.
• Experience withHCHB EMR (preferred but not required)
