Job summary

Transition Home Healthcare seeks a qualified CNA to provide in-home care across designated service areas in Texas. This PRN position supports the nursing care team and delivers compassionate, patient-centered services to patients and families.

Responsibilities include assisting with daily living activities, documenting changes, communicating with the care team, and maintaining accurate notes. Texas CNA license and one year home health experience preferred.

Qualifications

• CNA license in Texas is required.
• Minimum 1 year of home health experience.
• Ability to lift up to 50 pounds.
• Able to work weekends, holidays, evenings.
• Strong communication and people skills.
• Experience with HCHB EMR preferred but not required.

• Provide exceptional care in coordination with the assigned nursing care team.
• Maintain patient records that include changes in doctor’s orders, diet, or personal living activities.
• Help with patient meals, feeding, and activities of daily living.
• Deliver weekly reports to the case manager and review important changes in patient conditions.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with healthcare providers and other care team members.

CNA (Home Health & Hospice)

Shift Available: PRN

Service Areas for Coverage: North, Northeast, Northwest, Southwest
