Provides personal care services under the direction of the RN/Case Manager. Assigned to specific clients and performs services to maintain personal comfort.

Responsibilities

• Read and record temperature, pulse, and respiration.
• Patient hygiene – bathing, shampoo, skin/nail care, oral hygiene, shaving, dressing, toileting.
• Maintain a clean, safe, and healthy home environment – meal preparation, laundry, light housekeeping.
• Follow written assignments and instructions.
• Complete and submit appropriate documentation.

• At least eighteen (18) years of age.
• Current CPR certification and TB test.
