Job summary

Professional Case Management seeks a compassionate CNA to provide in-home personal care under the supervision of the RN/Case Manager. You will support clients with daily activities, meals, and hygiene, and perform skilled transfers with vital signs monitoring and ROM exercises.

Qualifications

• Active CNA certification in the state(s) of practice.
• Current CPR certification and TB test.
• Food Handler's card.
• Experience with skilled transfers, vital signs, timely documentation, ROM, and activities of daily living.
