Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. CNA/Home Health Aide

Service Areas : North Houston, Clear Lake & Galveston

Responsibilities:

• Provide exceptional care in coordination with the assigned nursing care team.
• Maintain patient records that include any changes in doctor’s orders, diet, or personal living activities.
• Help with patient meals, feeding, and basic activities of daily living.
• Deliver weekly reports to the case manager and review any important changes in patient conditions.
• Educate patients and family members.
• Prepare timely, complete, and accurate clinical notes.
• Communicate effectively with healthcare providers and other care team members.

Qualifications:

• CNA license in the state of Texas (required).
• One (1) year home health experience.
• Able to lift up to fifty (50) pounds.
• Able to work occasional weekends, holidays, evenings.
• Strong communication and people skills.
• Experience withHCHB EMR (preferred but not required)
