Job summary

Ecumen Home Care-Lakeshore is hiring a Home Health Assistant (CNA) to provide compassionate in-home care in Duluth, MN. The role focuses on one-on-one client interactions, personal care, and adherence to care plans in accordance with Home Care policies.

The position offers part-time or full-time options, day shifts, and a benefits package including PTO/Sick time and mileage reimbursement. Dedicated, supportive team environment with a focus on client well-being.

Qualifications

• Must be at least 18 years old.
• Certified Nursing Assistant (CNA) and satisfactory test out of the HHA component.
• Current valid driver’s license and proof of vehicle insurance (if applicable).
• Ability to communicate effectively in verbal and written formats.
• Adherence to safety policies and procedures.

Responsibilities

• Provide personal care per the client’s care plan and promote independence.
• Observe, document, and report changes in client condition.
• Communicate with client and family during in-home care; participate in care conferences.
• Prepare meals as directed, assist with light housekeeping and laundry as needed.
• Respond to safety hazards and emergencies.

The Home Health Assistant - CNA is responsible for providing personal care and other designated health-related services designed to maintain the client's physical and emotional well-being, in compliance with the Home Care policies, procedures and standards for the Home Health Assistant - CNA.

Opportunity Details:

• The targeted pay range for this job is $21.50 - $23.50 per hour.

Essential Job Responsibilities:

1. Provides personal care to clients per client's individualized care plan. Promotes independence with Clients, offering choices and fostering self-help skills.
2. Observes, documents and reports changes in physical, cognitive, and emotional condition of clients.
3. Communicates with client and/or family members while in client's home as appropriate. Participates in care conferences with other health team members as needed
4. Prepares meals as directed and requested, following special meal plans if instructed. May provide light housekeeping and laundry services as directed by the care plan.
5. Responds appropriately to safety hazards, fire and other emergency situations.

Home Health Assistant Minimum Required Qualifications:

• 18 years of age or older
• Licensure/Certification Required: Certified Nursing Assistant and satisfactory test out of the HHA component
• Current valid driver's license and proof of vehicle insurance (if applicable)
• Ability to communicate effectively in both verbal and written formats
• Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, MSDS

Preferred Qualifications:

• High school diploma, GED, or higher
• Previous experience working with seniors
