Job summary

Aria Healthcare in Wisconsin seeks a Certified Nursing Assistant (CNA) to provide daily nursing support and resident-centered care. The CNA will assist with ADLs, report health changes to the nurse, and participate in team-based care to ensure safety and comfort for residents.

Current Wisconsin CNA certification is required; strong teamwork and compassionate care are essential. This role is on-site, within a facility dedicated to high-quality resident care and safety.

Qualifications

• Must have current and active nursing aide certification in Wisconsin.
• Ability to provide resident-centered care.
• Teamwork and coordination with interdisciplinary staff.

Responsibilities

• Provide routine daily nursing care and services according to care plans.
• Assist residents with activities of daily living (ADLs) such as mobility, grooming, dining, bathing.
• Monitor residents, report changes to supervising nurse.
• Maintain infection control and safety practices.
• Record observations accurately in charts and logs.
• Collaborate with team to ensure high-quality resident care.

The primary purpose of a Certified Nursing Assistant (CNA) is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by your supervisors.

• Resident centered care oriented including assistance with activities of daily living such as:
• Positioning, Moving, and Restorative Care.
• Mobility, Toileting, and Bathing.
• Grooming: Oral, Shaving and Nail Care.
• Dining Assistance.
• Dressing and Undressing.
• Infection control and prevention.
• Nutrition and Elimination.
• Comfort care and End of Life Care.
• Answer call lights timely and promptly.
• Report all changes in the resident's condition to your assigned nurse as soon as possible.
• Report all accidents and incidents you observe on the shift when they occur.
• Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner.
• Be a team player working in coordination with all disciplines in the facility.
