Diablo Valley Post Acute in Concord, CA is hiring compassionate CNAs for full-time roles. You will assist with activities of daily living, follow care plans, and document care while contributing to a 5-Star Medicare-rated facility known for resident-centered care.

Responsibilities

• Provide compassionate, high-quality care while promoting residents' dignity, comfort, safety, and independence.
• Assist residents with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, mobility, and meals.
• Follow individualized care plans and document care and required records.
• Observe residents for changes and report concerns to the nursing team promptly.
• Help with transfers, repositioning, ambulation, and mobility needs.
• Communicate effectively with residents, families, and the care team.
