Job summary

Wabash Center is seeking a dedicated Certified Nursing Assistant (CNA) to provide direct personal care in community-based residential settings in Indiana. You will assist with ADLs, monitors health status, and support individuals in maintaining independence while following care plans and regulatory policies.

The role entails medication administration under supervision, incident documentation, and collaboration with nursing staff, house management, and program teams to ensure safe, quality care

Qualifications

  • Active CNA certification in good standing with ISDH.
  • High school diploma or GED required.
  • Minimum 1 year CNA experience preferred; residential care valued.
  • CPR and First Aid certification current or obtainable within 30 days.
  • Successful completion of medication administration training within required timeframe.
  • Background check clearances per state requirements; valid Indiana license.
  • Valid Indiana driver’s license and reliable transportation.

Responsibilities

  • Assist with activities of daily living (ADLs) per care plan.
  • Monitor vital signs and document health indicators as directed.
  • Administer medications per policy and Indiana regulations.
  • Observe and report changes to nursing staff and management.
  • Provide transfers, repositioning, and ambulation support.

The Certified Nursing Assistant (CNA) in Supported Living provides direct personal care and nursing-related support to individuals with disabilities living in community-based residential settings. Working under the direction of nursing staff and house management, this position assists with activities of daily living (ADLs), monitors health status, and supports individuals in maintaining independence and quality of life in their own homes. This role combines DSP responsibilities with CNA clinical skills to meet both habilitative and health-related needs.

ESSENTIAL FUNCTIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

1. Assist individuals with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, mobility, and feeding, according to their person-centered individual support plan (PCISP) and risk/wellness plan.

2. Monitor and document vital signs, weight, intake/output, and other health indicators as directed by nursing staff and the risk/wellness plan.

3. Administer medications in accordance with Wabash Center’s medication administration policies and applicable Indiana regulations.

4. Observe and report changes in individuals’ physical, behavioral, or emotional condition to nursing staff and house management promptly.

5. Provide hands-on support with transfers, repositioning, and ambulation using proper body mechanics and assistive devices.

6. Assist with meal preparation and feeding support, following any dietary plans or restrictions outlined in the individual’s care plan.

7. Maintain accurate and timely documentation of care provided, health observations, and incidents per Wabash Center and FSSA/BDDS requirements.

8. Support individuals in building and maintaining daily living skills, community participation, and personal goals as described in their PCISP.

9. Respond to medical emergencies and provide first aid or CPR as needed, following established emergency protocols.

10. Perform light housekeeping tasks in the supported living home, including laundry, cleaning, and maintaining a safe living environment.

11. Transport individuals to medical appointments, community activities, and other scheduled outings as assigned.

12. Communicate with the care team — including nursing, house management, and program staff — to ensure continuity and coordination of services.

13. Promote the health and safety of all individuals through adherence to all Wabash Center and departmental policies and procedures, including protecting individuals from abuse/neglect.

14. Maintain all appropriate information in a confidential and professional manner in accordance with state and federal rules and regulations.

15. Actively participate in department and/or service site discussions and meetings as needed and take action on all assigned items.

  • Earn an extra $3.00 per hour for eligible 2nd- or 3rd-shift CNA hours.
Requirements
QUALIFICATIONS

1. Active Certified Nursing Assistant (CNA) certification through the Indiana State Department of Health (ISDH). Must remain in good standing on the Indiana Nurse Aide Registry.

2. High school diploma or GED required.

3. Minimum one (1) year of CNA experience preferred; experience in residential or supported living settings strongly preferred.

4. Current CPR and First Aid certification (or ability to obtain within 30 days of hire).

5. Successful completion of Wabash Center’s medication administration training within the required timeframe.

6. Must meet all FSSA/BDDS requirements for direct care staff, including background check clearance per 460 IAC 6.

7. Valid Indiana driver’s license with acceptable driving record and reliable transportation.

8. Ability to work a flexible schedule, including evenings, weekends, overnights, and holidays as needed for residential coverage
