Job summary

Interim HealthCare of Walnut Creek is seeking Certified Nursing Assistants (CNA) to provide compassionate home-based care across Alameda and Contra Costa Counties. You’ll deliver hands-on support, monitor vitals, assist with mobility, and help patients stay safely at home.

Qualifications

• Must have CNA license in CA and Home Care Aide registration
• Current CPR certification
• TB or PPD test results current
• Ability to pass required background, health, and employment screenings
• Reliable transportation to patient homes
• Two positive references from previous employers or supervisors

Responsibilities

• Provide hands-on personal care including bathing, grooming, oral hygiene, toileting, and skin care
• Check and record temperature, pulse, and respirations
• Assist patients with ambulation, transfers, and ROM exercises
• Support daily living with meal preparation, nutrition, and a clean, safe home environment
• Give medication reminders and follow care plan limitations
• Follow infection prevention practices and respect patient rights and dignity
• Document visits accurately and report changes to supervisor

What You'll Do

• Provide hands-on personal care, including bathing, grooming, oral hygiene, toileting, and skin care

• Give medication reminders and perform only the tasks assigned in the care plan
• Follow infection prevention practices and honor each patient's rights and dignity
• Document your visits accurately and report changes in condition or safety to your supervisor

What You Bring

• Active and unencumbered CNA license in the state of CA
• Home Care Aide registration required to provide personal care in CA
• Reliable transportation
• Current CPR certification
• Current Annual TB orPPD
• Two positive references
