Interim HealthCare of Walnut Creek is seeking Certified Nursing Assistants for home care across Alameda and Contra Costa Counties. The role offers Full-Time, Part-Time, and Per Diem positions with competitive hourly pay and flexible scheduling to fit part-time needs.

CNAs provide in-home personal care, monitor patient conditions, assist with daily living activities, and document visits.CA license, Home Care Aide registration, CPR, reliable transportation, and references are required.

Qualifications

• Active and unencumbered CNA license in CA is required.
• Current CPR certification required.
• Home Care Aide registration in CA required to provide personal care.

Responsabilités

• Provide hands-on personal care (bathing, grooming, oral hygiene, toileting, skin care).
• Record vital signs and monitor patient condition.
• Assist with mobility and transfers; ROM exercises.
• Assist with meals, nutrition and home environment cleanliness.
• Administer medication reminders per care plan.
• Follow infection prevention practices and respect patient rights and dignity.
• Document visits and report changes to supervisor.

What You'll Do

• Provide hands-on personal care, including bathing, grooming, oral hygiene, toileting, and skin care
• Check and record temperature, pulse, andrespirations
• Help patients move safely with ambulation, transfers, and range-of-motion exercises
• Support daily living with meal preparation, nutrition, and a clean, safe home environment
• Give medication reminders and perform only the tasks assigned in the care plan
• Follow infection prevention practices and honor each patient's rights and dignity
• Document your visits accurately and report changes in condition or safety to your supervisor

What You Bring

• Active and unencumbered CNA license in the state of CA
• Home Care Aide registration required to provide personal care in CA
• Reliable transportation
• Current CPR certification
• Current Annual TB orPPD
• Two positive references
• Compassion, dependability, clear communication, and the ability to meet the physical requirements of the role
• Ability to complete all required background, health, and employment screenings
